Population Health and Wellness - HSO 60001:2018(E)

The Population Health and Wellness standard is designed to be applied to a priority population, or multiple priority populations, identified by the organization. Priority populations could be condition-specific (e.g. mental health or cancer), or age-specific (e.g. children and youth or seniors). An organization may select its priority population(s) based on priorities set out by the regional government, or the organization’s own strategic plan and priorities.

The standard for Population Health and Wellness is based on:

  • Wagners et al’s Chronic Care Model,
  • the Expanded Chronic Care Model,
  • input from an expert multi-disciplinary standards working group, and
  • a review of research and best practices.

Many services promoting population health are delivered by primary care providers in the community. These standards address the importance of primary care, the need to align acute and tertiary care to support the services provided by primary care providers, and the importance of integrating services across the continuum of care.

The standard contains the following sections:

  • Investing in population health and wellness services
  • Integrating and coordinating health services across the continuum of care
  • Basing decisions on research and evidence
  • Using information and information technology
  • Monitoring quality and achieving positive outcomes

HSO is an independent, not‑for‑profit corporation and is registered in Canada as a charity, in accordance with Canada Revenue Agency rules, with a mandate to enable and empower people around the world to continuously improve quality of care.

While designated as a Standards Development Organization by the Standards Council of Canada (SCC), HSO operates independently from SCC. HSO does not receive ongoing funding from SCC, other crown corporations of Canada, or federal or provincial entities.

HSO charges a nominal fee to help offset the associated costs of development and maintenance of HSO standards, as most HSO standards are self-funded.

Standards available for purchase through this E-Store are considered HSO standards and must only be used in accordance with applicable HSO Terms of Use or client agreement and are not applicable for assessment purposes. If you are part of an assessment program such as the Qmentum accreditation program, with one of our partners including Accreditation Canada, Qualicor Europe, IQG, or IQMH, your assessment body will provide you with the assessment standard prior to the standard being incorporated into your program and used for surveys or assessment purposes.
