About HSO Standards
HSO standards are the foundation on which leading-edge accreditation programs and evidence-informed public policy are built. Standards create a strong people-centred health care system that interest holders can rely on to enable people around the world to continuously improve quality of care.
HSO standards are formatted using the following structure:
- Section title. A section of the standard that relates to a specific topic.
- Clause. A thematic statement that introduces a set of criteria.
- Criteria. Requirements informed by evidence that describe what is needed by people to achieve a particular activity. Each criterion outlines the intent, action, and accountability.
- Guidelines. Additional information and evidence to support the implementation of each criterion.
HSO standards integrate gender responsiveness though three key mechanisms:
- Diverse representation. The Technical Committee for each standard includes a diverse range of genders and professional backgrounds. This composition supports the inclusion of lived experiences and clinical perspectives on gendered differences in care delivery that are central to the development process.
- Evidence-informed research. HSO Standards draw on research that explores gender-based differences in health issues and care delivery. This includes examining variations in prevalence, symptom expression, access to and experiences of care, treatment, outcomes and service standards.
- Inclusive language. HSO standards use inclusive gender-responsive language that respects diverse gender identities and reflects both physical and physiological as well as social and cultural dimensions of gender.

About This Standard
CAN/HSO 3001:2026 (E) Medication Management is a new edition of the CAN/HSO 3001:2019 (E) Medication Management standard. This new edition incorporates and supersedes the content from CAN/HSO 34014:2019 (E) Medication Management for Community-Based Organizations, CAN/HSO 5030:2020 (E) Antimicrobial Stewardship
Program, CAN/HSO 11013:2022 (E) Cannabis Use for Medical Purposes: Inpatient Care Settings, and HSO 51001:2018 (E) Community Pharmacy Services.
The standard was developed using published evidence and input from diverse interest holders, including people with lived experience. Gender-disaggregated data was considered during the literature review. Additionally, gender responsiveness was addressed through inclusive consultation and integration of equity, diversity, and cultural safety principles. Physical, physiological, and social gender considerations informed requirements for individualized care and decision-making. The technical committee included experts and users from multiple sectors, and the scope spans all populations and care settings, embedding gender equity throughout the technical content.

This standard is intended to be used as part of a conformity assessment. This standard will undergo periodic maintenance. HSO will review and publish this standard on a schedule not to exceed five years from the date of publication.
Scope
Purpose
This standard provides guidance for all activities related to establishing a medication management program, prescribing, preparing, storing, dispensing, administering, monitoring effects, and disposing of medications. It applies to all medication types and modalities, such as controlled substances, medically authorized cannabis products, investigational and special access medications, compounded products, and complementary and alternative medicines. The standard also includes requirements for antimicrobial stewardship, deprescribing, and environmental sustainability.
This standard specifies the accountabilities of organizational leaders, the clinical team, the pharmacy team, and the team that includes the client and their care partner(s) as integrated members in collaboratively enabling safe medication practices and reducing medication-related hazards, close calls, and incidents across all care settings for all populations.
This standard does not prescribe clinical practice guidelines, dosing requirements, or technical guidelines. Rather, it seeks to embed people-centred care principles into medication management practices and promote the translation of evidence into action by encouraging continuous learning for regulated health care professionals and unregulated health care providers. The standard upholds a commitment to equity, diversity, inclusion, and cultural safety; coordinated care and integrated services; and a quality-improvement and outcomes-focused culture.
Applicability
This standard applies to all health and social service organizations with a role in medication management.
The defined population for this standard is all ages.
HSO is an independent, not‑for‑profit corporation and is registered in Canada as a charity, in accordance with Canada Revenue Agency rules, with a mandate to enable and empower people around the world to continuously improve quality of care.
While designated as a Standards Development Organization by the Standards Council of Canada (SCC), HSO operates independently from SCC. HSO does not receive ongoing funding from SCC, other crown corporations of Canada, or federal or provincial entities.
HSO charges a nominal fee to help offset the associated costs of development and maintenance of HSO standards, as most HSO standards are self-funded.
